Our Driver Volunteers provide valuable support to Blesma, by collecting and delivering equipment to and from various destinations. You will be required to lift and carry heavy or cumbersome items and so you must be physically able to do so.

By signing this document you are confirming that
- You consider yourself fit to undertake the role of Driver Volunteer
- You are responsible for managing your own limitations and restrictions
- You will notify Blesma if your health changes and you no longer consider yourself able to carry out the role
- That you have a vehicle that is suitable for transporting equipment.
- You have, or will, complete the Driver Declaration in conjunction with this form
